Fellaini played youth football for Anderlecht, RAEC Mons, Royal Francs Borains and Charleroi SC before joining Standard Liège. On the international scene, Fellaini has been a full international for Belgium since 2007, with more than 80 caps. He represented the team at the 2008 Olympics, 2014 FIFA World Cup, UEFA Euro 2016 and 2018 FIFA World Cup.
He was eligible to play for Morocco or Belgium and he chose Belgium, starting at youth level. He represented the Under-23 team at the 2008 Olympics when they finished in fourth place.
